DESCRIPTION:Health forms and all Camp InTouch profile forms are due no later than May 15\, 2024. Please inform the Director if your camper cannot complete the Health Provider form until after this date due to insurance or scheduling issues. If there are any health updates including changes to medications\, please update the Healthcare Manager and/or Director at the earliest possible opportunity. In the event that this deadline is not met\, Camp Carysbrook reserves the right to assess a late fee. \nCamper health and safety are key to a summer to remember and your camper’s success. We review these forms to prepare to best support your camper and partner with you to maximize her experience and give you peace of mind while she is in our care. \n
